For new enrollees:

                                  Form 138-A Report Card

                                  Good Moral Certificate

                                  Birth Certificate (Xerox Copy)

                                  2 pcs. 1x1 ID Picture

                                  1 long brown envelope

For transferees:

                                  Honorable Dismissal and/or Waiver

                                  True Copy of Grades

                                  Good Moral Certificate

                                  2 pcs 1x1 ID Picture

                                  1 long brown envelope

For modular/short courses:

                                  1 pc. 1x1 ID Picture (1any valid ID)

                                  1 long brown envelope
